DEVICE APPLICATION FORM
Parents, please fill out the application form. Once we receive your application, we will email your speech therapist to verify that they will be supporting you through the process of learning to use your new communication device.
After verifying with your therapist, your child's name will be submitted to our committee. After ensuring that all application requirements are met, the selection process is completed through random selection as we know all children are equally deserving of the right to communicate.
